The fish has swimbladder problems. This is quite common in fancy goldfish.

There are many causes, I suggest you work through the following:
1. Bad water - check your water parameters, do pwc as needed to correct. <I would do a pwc on spec even if parameters are normal, as there may be bad stuff (eg organics) in the water that is accumulating, but not measured by your kit.
2. Gut problem - a constipated fish, or one with gas in the gut will not be able to swim properly. First, make sure the fish is not too cold ... a fancy goldfish will have far more gut problem if kept in cold water (like low 60's), and all goldfish should be fasted when water temp is in the 50's. <They should be preparing for hibernation at those temps.> Feed peas - shelled, blanched & chopped. Fancy golds should have veggies regularly. When constipated, peas will help, and this should be the second thing to try in swimbladder problem. <Right after checking water quality.> To prevent future problems, feed sinking pellets & veggies.
3. Internal infections - either bacterial or parasitic - may cause swimbladder problems. If all else fails, trying a course of anti-parasitic food (eg Praziquental) and a course of anti-biotic food (eg Romet, Kanamycin) is worthwhile.
4. Congenital - some fancy goldfish have irreversible SB problems. If the fish is otherwise healthy, you can leave things be. there are surgical procedures to correct this if the fish is valuable. <Look at KoiVet.com for some ideas ... you will have to find a fish vet for this.>
