Any ideas why my amazon swords new leaves are coming up twisted and damaged?

Ya it's had 2 seachem tabs under it for a month and a half now. Also dose flourish comp and trace. Excell for co2
 
Twisted leaves can happen when there is a calcium deficiency or too much magnesium or too much potassium. Do you know your gh and kh levels?
 
Well I'm at work right now and can't recall which one was which but one took 8 drops and the other took 12 drops. I have very hard water where I live. Tap has high ph also around 8.2.
 
But about a month and half ago I took my tank down and switched from gravel to eco fine grade and it was growing good straight leaves before the changeover.
 
Then I would cut off the new twisted growth and perhaps add another root tab. If you disrupted the roots a lot when you replanted this could have disrupted new growth coming in. Once you cut off the bad growth see if new comes in properly.
