Oh ok, it'a a planaria worm. Harmless but annoying. Your fish will eat them, but they can multiply quickly if you overfeed and don't clean the gravel well.

how do I clean the gravel properly in a planted tank? and I don't have fish... just shrimp

Just squeeze between and around the plants as best as you can.

Feed less, water changes, don't stress.
If needed you can use a dog dewormer that won't harm your Shrimp.

how often should i do water changes now that that thing is in it? and what % would you suggest for a five gallon?

Not too much for shrimp. Maybe 10-20% a week.
