Would I have to vacum the sand in my 46 gallon cichlid tank or is tank out water just fine?

Yes you need to vac the sand. Just hold your siphon like a half inch above the surface, you may have to give it a little stir but the poop will come up

Is there any way to make a DIY python of some sort as my old vacumm is very small

For sand, the smaller the better. Ime. I have even seen people on here recommend using airline tubing for sand. I guess it gives you more time to remove organics before too much water comes out.

I mean the tank is to tall so my housing can't reach the bucket and I will try to get a thinner acrylic hose do you know a cheap place to get it from

I'd check at Lowes hardware. I have bought all sorts of tubing and hoses from there for way cheaper than any lfs.
