Percula Clownfish Died !!!

Hello to all does anyone know why my clown turned white and gasping for air than it just lost all swimming abilities and just died?
bought her with her mate, about 3 months ago and she look very healthy.
I am so sad cause she was a beautiful one to look at.......I am concern about the rest though. anyone please chime in!!!

parameters:
pH:7.8
ammonia: 0
Nitrite: 0
Nitrate: 40
Calcium: 440
Posphate: .25

3 clowns (2 now), 2 damsels, 1 cardinal, 2 Green Chromis,
 
Nitrates are high, I would get that down with PWC's ASAP. Is it possible that the other two were picking on it?
 
I dont know for sure, but im guessing the other two clowns killed it. You should only really have 2 clowns per tank max because the pair can team up on the third. Any agression issues?
 
I got her as a pair, no fighting what so ever, in fact she was the dominant one of the three clowns, one was her mate and the third was really too small to even try to pick on anyone.
