Betta's head red and peeling

I've had this betta for just over 2 months. Her head was red when I got her but I feel like it has spread a little and the scales on her head look like they're peeling. She is eating and acting normal. 30 gallon tank, also 2 months old with Marineland penguin double filter, 300 gph. Pics attached: the first one is when I first got her. Water parameters:
pH- 7.5
Ammonia- .25
Nitrate- 40
Nitrite- .25
Alkalinity- 180
Hardness- 150
I do about a 30% water change every Sunday, so I know my nitrates and nitrites are questionable because I haven't done it yet this week. Community tank with 15 other fish, mostly glofish.

i would have to say with all the little holes in the head or what looks like holes its hole in the head disease. i could be wrong though.

I second goatnad. Looks like hole in the head. I would do 10 gallon water changes every 3 days and treat with Seachem Metroplex.
