I've got a small red cherry shrimp tank with ~30 RCS (mostly juvenile) and a good deal of java moss. The tank has been set up for several months now. Today, I saw a bunch of thin, white worms on the wet glass just above the water line. I also saw what I assume were their eggs. Help!

1) Where did they come from?

2) How do I get rid of them?

3) How do I prevent them from ever coming back?
 
Likely planaria. They aren't harmful but usually are an indicator of dirty substrate. You can get rid of them by doing a water change and siphoning out your substrate.
 
They can also appear if you overfeed your fish. I have had them and still have a few now. The fish may eat them.
