I've never heard of a freshwater damsels, I'm interested in what it looks like. Also, what are your water parameters, I'd say if all your fish died then your tank is not cycled, and you'll want to look into fish-in cycling.
They are extremely dirty fish that put off a ton of waste. I have 3-3inch commons that I do 2-25% weekly water changes on. And that's with a canister, and a 55 gallon tank, which is still on the very small side for them.

The high ammonia causes gills to burn, and would be your likely cause of flashing. I'd suggest a PWC to keep it in line until you figure out the source.

I have a yellow belly cooter and he can get pretty stinky. I use a canister filter and it's amazing! But if your stuck with the hang on then I'd try to add another, the general rule is cut filter rates in half. And being as it's a turtle tank, they are extremely dirty critters.
