It looks like a fungus or a parisite most likely came in with the fish or from your tap water. I'm no expert and I would suggest if no one on here can identify it, take the pics to a aquarium expert or sales store and ask someone with the most experience with fresh water fish diseases. Yea a sponge will do, just make sure it is thin or as thin as you can get it. Thick sponges will slow the water flow.

There is also a plast mesh you can cover your filter intake with, nylon is not so porous so the water is having trouble getting through. I used it for my wave runner so my cna and baby fish wouldn't get stuck or sucked in ;)

Try using blood worms and brine shrimp, that is what I use for my rainbows. The worms absorb water quick so they are easy for top feeders like rainbows.

Yep they are escape artists, and can survive out of water for like an hour. I would always have a top for my tank so no one gets out.
