I would recheck the levels and then dose the ammonia to 4ppm and then retest 24 hrs later there should be a change in nitrites.
Another option is a water change in case the cycle stalled.
I may be wrong but as far as I know it doesn't consume the fish it just burrows in the skin. It's possible that the fish has a fungus or another type of disease. Can you get a picture of it?
I think the simple green or clean can't remember :) should help remove them. I know CLR would remove the stains but I doubt it's safe to use in an aquarium.
Ghost shrimp are good scavengers but depending on the area you may only encounter bad stock. In my area they are mainly used as feeders so they are not well cared for. Try aqua bid they have many shrimp choices and the ones I've purchased were very nice.
I agree it will most likely take some time to see a spike of ammonia.
Do you know what your ph is? Driftwood lowers ph and when ph drops too low the bacteria cannot do what it needs to.
