    Update: wife was able to find the rest of the cardinal tetras, so I guess they were just hiding really well. I have asked her to turn the CO2 all the way down to about 1 bubble every 5-10 seconds. I'm otherwise planning on doing the rest of the above unless anyone has any other suggestions.

    This is my concern - unless pre-cycled media was added, I highly doubt a tank was cycled in ~1 week. Then again, if it wasn't cycled and fish have been in there for ~2 weeks, I would expect the parameters to be more abnormal.

    As it comes, it would probably be a problem if your fish are quite small. However, you could fix this pretty easily I think: either by adding a sponge at the end, or by rigging some kind of mesh to surround the actual intake. The former would probably be easier.
