Ghost Shrimp turning yellow and dying

I have noticed discoloration in my ghost shrimps for a while (maybe a month). Their legs are also becoming slowly misshapen. I ignored it for a while, but two of them died this week.

Cherries, amanos, and Panda Corrys are also in the tank, and appear totally healthy.

What is this? Bacterial infection?

Do you top off water with RO or distilled or treated tap water?

Do you use any supplements /additives?
 
I feed them aqueon tropical flakes, hikari shrimp cuisine, algae wafers, frozen bring shrimp, and bananas.

GH ~80
KH ~20
PH 7
Nitrites 0
Nitrates ~20

Treated tap water
No additives or supplements

I hit them with 5 ml hydrogen peroxide for 3 days and it cleared up, however.
