Help please!! Dojo loach tail

My dojo loach has been floating at the top of the aquarium and hasn't been eating for about 2 days. I observed it to notice a cut or rip on its tail. I've attached a photo, has anyone had this problem? How can I help it?

Clean (dechlorinated) water in the aquarium and lots of it would be my first thought.

Are you familiar with the nitrification cycle?

How long has this tank been running,

and what is your maintenance schedule?
