What do my test results mean and what should I do about it?

Pardon my ignorance but this is my first time using the kit. It appears my water is less than ideal for my 5 platies and clown pleco but I'm not sure where to go from here. Is it time for a pwc or will I require more than just that?

Your nitrites look very high I would do a water change 50%. It looks like nitrates are forming so the tank is in the cycling process but it might take a few weeks to fully cycle.

You will have to do a high ph test it is above 7.6

Nitrates look very high too this could harm fish it shouldn't be above 40

What size tank and how long as it been set up?
I agree, I would do a few 50% water changes a few hours apart to get those levels down, particularly the nitrites (don't forget dechlorinator). These links might help some;
