How much you add and how often will depend on what your test results tell you. Always always always test for what you're adding.

But like Mike said, on a 12g, normal weekly water changes should replenish all the elements you need.

Any corals. Maybe depending on how many SPS corals you might need to add calcium but your test results will let you know.

There u go. Test water, then add. You will be surprised that your water changes keep you full on most nutrients. Are you doing weekly water changes?
