I have been treating, for what I thought, was ICH for 4 weeks now. Only did heat and salt. Made no difference. So I started adding Paraguard by Seachem and no difference. Maybe it is NOT ICH??
Could be normal spotting pattern he is developing?

10 gallon
1tsp salt to 1 gallon water
- syno cat with spots
- healthy dwarf puffer
Temp 85

Are they suppose to get white spots? My upside down cat has the same thing but I don't know about synod

Isn't ICH and white spot disease same thing ?

Ich and white spot are the same thing. Temperature alone can clear up an outbreak of ich. Temp needs to be at least 31deg C (88deg F) and leave it there for at least a week after last spot disappears.
Make sure you've got adequate aeration as the hotter water holds less oxygen.
 
Yes . 2 air stones. Had temp at 85 + salt now for 4 weeks and no improvement. Started adding Paraguard once a day. Not sure about dosage?

Is it normal for white spots on syno ? Just the way they are.....
Upside down cats have normal white spots that appear as they age

Everything I've read is very specific about temp needing to be higher than 31C(88F). I know you said 85F+ but what exactly is your temp?
 
There is a dwarf puffer in the tank as well and both were very uncomfortable at that high of temperature , so had to turn it down
- but the Paraguard should take care of it ??
 
I put a digital thermometer in the tank and it said 82 degrees
- I'm gonna go the medicine route and keep up with Paraguard
