Ammonia and Nitrite

After doing a 50% water change, it doesn't seem to have helped.

My ammonia stayed the same but my Nitrite jumped to .50.

Prior to the water change, they were both reading at .25, hence the reason for the water change. Is prime causing the higher readings?
 
Also, after the water change, one of my Gold Tetra's has been hanging out by the filter. I did wash the filter media, bit that was in old tank water.
 
Its a 29 gallon, 5 gold tetras and a bn pleco. I tested about 10 min. Also, after doing the water change, I noticed something odd at the roots of one of my plants. Could this be a culprit?
