You might also try calling your local glass store. The one in our town will cut glass to the dimension we need, as well as cut notches for cords and such to go through. I don't know if they can do hinges, though. Depending on what shipping charges are it might be worth calling around a bit to compare prices. (I'm also incredibly impatient and love the next-day service from our local store!)

Places mentioned above are where I get mine. Both stores pack them very well so you shouldnt have to worry about a broken top.
