sick colt coral?

I have a colt coral that I believe is sick. It looks as if fish have been taking bites out of him but they are not. The coral has about 6 white "bite" marks and every couple days another one will appear. Evetually the finger turns a darker pink and slowly gets worse until the finger is hanging on by a tiny "string". That is when I pull the bad looking finger off. Pretty soon he is not going to have any fingers left at this rate. My other corals and plants are doing perfect. What is going on? Is it best to cut the finger off with a razor blade right when I see these white gashes or should I wait until they are just about to fall off and then pull it off? This poor guy just doesn't look very happy. His little "feelers" come out still but not all of them, only about 1/2. Please help. Any info. would be helpful.

What kind of fish and inverts do u have
what type of lighting do u use
what are the water parameters
ammonia
nitrite
nitrate
salinity
ph

also just curious what are the "plants" u speak of
 
sounds to me like it is self-fragging. Are the little pieces trying to attach to rock?
