Sinking coral??

I came home from work today to find one of my mushrooms has had a hole dug under it? Also a couple other holes around the tank. These are the best pictures I could get. Livestock is 2 Occ clowns, 1 green Chromis, one law mower blenny and a wild caught goby. Does anybody have any idea what could have dug these holes?

He has been in the tank for 2 months and only stays in the rocks. The other day I noticed a tube like worm in my sand against that glass could that be what is making the holes? The worm was stretching and expanding and had little tentacles and the top that was pulling sand into its body

Finally was able to get a half way decent photo of the worm that lives in my sand bed
