Not too sure. If you tell us how she is acting and such symptoms it would help tremendously. Just posting a picture doesn't help that much.
Is it alive or hanging like poop? I would go for some parasite treatment and/or maybe a medicated food for worms. Not sure they can be combined. Water changes and good diet. Good luck! Others in this forum have some great experience and knowledge to help!
Well she likes to hangout near the surface of the water alone. If other fish come near her she doesn't move far from her spot. It doesn't appear that she is eating at this time nor is she using her back tail fin to swim anymore either. The long white thing coming out is just getting longer and it appears to no movement on its own. I hope this helps.
