Generally things like that are caused from stress, weakened immune system, or unclean water. All three are usually linked together. I am not sure what is on his head. Wether its an abnormal growth or natural. What are your tank parameters? Ammonia, nitrate, and nitrite readings from a liquid test kit. Tank size, what fish are in the tank, and how long the tank has been set up?

the water is crystal clear it was set up for about a year before i resently moved i keep water out of the tank with the fish in the bucket with a battery operated pump when i moved and added the water back to the newly set up tank it been good going for a month now havent checked the water yet since i moved what i also did noticed after the pleco passed that the brown algae has came back

20hg tank
i got 3 corys and
recently added 2 swordtails
and had my pleco :( i liked that pleco

I would do a 50% water change regardless to dilute a possible cause. Its the first thing i do when sickness or in this case deaths occur. Cant see whats growing from its head but its not good.

Crystal clear means nothing. Water thats clear can contain anything. How often and how much water are you changing?

i will do the water change in the morning since its late here i do 50% waterchanges every 2 weeks and i still cant believe the pleco was fine yesterday healthy as can be and now dead :(

Those are just natural spikes they use to defend territories. There is nothin wrong with his gills...

One BNs die they extend these fully and it stays that way. Right now the only cause of death I can think of if the water params are perfect, is either poor diet, or lack of wood

+1...I have had one to die and they do extend those spikes for defense and mine also died with them extended as if she was fighting/defending herself

so the poor diet was my fault then when i feed my fish i was feeding them shrimp pellets but after adding the sword tails was tropical flakes so pretty much starved my pleco

and for some reason swordtail keep dieing after doing water changes and i make sure the water is right at 72°F everytime so im confused

Mix the prime before adding the waterit is supposed to work instantly but mixing and waiting 1 hour is good, 2-3 is possibly better iffish are dieing during WC. Add the primed water in small increments. If the swordtails are sensitive to chems in your tap water then adding prime before putting it in your tank is crucial.or add prime to your drained tank before adding water at least. Just options. I haven't kept swordtails so i don't know anything about them really. I a;ways premix my water conditioner before adding water to the tank and wait at lesast a few minutes.

oh ok i will add it to my drained tank since i use a hose that connects to vacuum from my faset how long do i wait after adding the prime to the tank before i put water in

I use an API product instead of prime but i assume prime works instantly as well. Have a read on the label to check if it says so. If you just add prime to the aquarium before you add tap water it should be ok assuming it dechlorinates etc instantly. Add enough for the complete volume of the tank, not just what you will remove for WC.
