Accidentally uprooted a plant

I accidentally uprooted one of my plants. How should I replace it into my tank without damaging the roots?
 
What kind of plant?

Generally inserting it back into the substrate at a 45 degree angle will help the substrate hold it in. You can use a tweezer if it is small. Ot just lift a scoop of substrate and angle it in the spot and drop the substrate over the top from an inch or so from the bottom.
