Glofish deformed, possible columnaris?

Is this more than columnaris and something else or columnaris at all? This Glowlight looks deformed in some way. Have had this fish about 3-4 months. Here are the parameters... Ammonia 0, NitrIte 0, NitrAte 5, pH 7.0. He is in tank 2 (see sig line). Pwc and vac 4 days ago done weekly. All other fish seem fine. As if it isn't enough I've also got a sick Betta (from a completely different tank) who isn't snapping back.

It may be columnaris, it may be a fungal infection caused by an bacterial infection. I would QT and treat with Maracyn.
 
Columnaris spreads with higher temperatures and is very very contagious. Don't use anything from this tank on another tank without bleaching it first, make sure you rinse it well and the last rinse use dechlorinated water.
 
Unfortunately died this morning. Was incredibly quick whatever it was (less than 24 hours). Upon closer exam this morning right before dying, it was very dark under the white fuzz on top of the head, gills were very red. Other fish seem fine today. Will keep a very close eye on them for the next few days.
