half and half wrasse white spots

I have a 2 inch half and half wrasse. Every once in awhile it has white spots on the black part of its body. They then very quickly disappear. There seems to be a correlation between this happening and its eyes being very open instead of being their usual black sunglass look. Also, it swims up to other fish, even my snowflake eel, and slowly rotates its body. Any help will be greatly appreciated.

Since it doesn't appear to be multiplying and no other fish are affected it doen't sound like ich. Could be a few grains of sand he gets stuck to him every once and awhile.
