I bought this plant at my lfs. No idea the name of it so I don't know how to care for it. Also I need to know if it can live with play sand as I am switching from gravel to sand

It's a crypt. Crypts can live in low light but I would recommend a nutrient ditch substrate. Or you could dose some liquid ferts.

Also do I have to leave it in that ugly "pot" from the fish store
 
Remove the plant from the pot and get as much of the rock wool that it's growing in off. If roots are longer than 2 inches trim them to that length to encourage new root growth after planting. Crypts are heavy root feeders so you will need to get root tabs and use them next to the plant. Crypts will use very little ferts from the water column.
