My fish died cant figure out why do you think it could be a shell that I put in there that's giving of something. I tested the water and its perfect

Ammonia/Nitrite/Nitrate readings?
How long have you had the fish?
Any odd visible markings or strange behavior recently?
What sort of shell is it?
Did you clean the shell before putting it in?

How about the rest of the answers to what azmodan posted? None of those are really aggressive so I would rule that out.
