What did I do wrong?

I have a Dalmatian Molly and she was pregnant with her second set of babies and was within a day of giving birth(judging by size) but she just died.

Could it have been something I did that caused her to die?

Note: her eyes seem to be bigger than usual and her scales look like their almost falling off.

I do not know much about the illness to help you, but I do know that she had "dropsy" googling that will yeild answers.

Yah, the raised gills and 'pop eye' is a bell ringer for bacterial infection..
