You can also mechanically remove it by wipin it away too. That'll only help in the short term though. They will go away eventually.
Leaves falling off could be a sign of light or nutrient deficiency. I had that problem and it was light based.

It looks like moss and dwarf hair grass. I think you'd need some pretty high light to keep the grass that low.
The tree looks like its formed by just tying some moss to driftwood and letting it grow and trim.

Thanks for the links sharpchick. The one thing that's not letting me say its just a behemoth of a ghost shrimp are these blue streaks. I'll try and get a better picture up later.
