yellow severum has scratches on face and keeps rubbing off plants

Hi guys, I'm john I have a severum who keeps rubbing off plants ornaments! When I have a good look he has scratches on his face? I've tested my water and my water is fine and other fish are fine? Dunno if it's some kind or parasite? Any help would be grateful, thanks

It would be helpful if you could post a photo of the fish, and give us readings in ppm for ammonia, nitrite, and nitrate. Any other info you can give us about your tank would be great too- tank size, tank mates, water change schedule, how long tank has been running, etc.
