Blue ram. Sunken belly-

Hi all. I have two blue rams In with my tetras and bristlenose, 120 liters, they re male and female... The female has gone quite dull on the last 3 months and has also developed a sunken pink tummy, she is interested in food but doesn't eat much. They started out the same size but now the make is twice the size of her. He also chases her quite a bit..
. I'm guessing Internal parasites.
I have treated with levimisole (don't think I spelt that write) - no change was noted.
Here's my questions
1- is it parasites?
2- can I put her in a 10 litre and treat her again ( or is a hosp tank that size too small?)

What are your water parameters? pH, Ammonia, nitrites, nitrates, temp?
 
Temp 27degrees
Ph 7.4-7.6
Amm 0.25 (did a big water change and vac yesterday, always has a spike for a couple of days. Our tapwater is quite high in ammonia ) usually 0
nitrite- 0ppm
Nitrate- 5ppm
 
Please note: pic was taken after a gravel vac and tank rearrange. That's why the water looks a little murky
