Calico moor with wound

I was wondering if someone could give me their opinion on this open wound- what it is and what we should use to treat it. We have used primafix fungal treatment and methalene blue antiseptic.

I’d go straight into antibiotics and something extra to clean up the wound. And water must be pristine, no ammonia or high temps. Having said that more details on cause would be useful.
