I was wondering if Ick always requires intervention.
Is a fish able to fight it off with just their own immune system and perfect water quality?
 
Yes and no. Generally speaking, ich is present in most systems, so if the fish don't have it, they're fighting it off. If they do show the parasite, that generally means they're not strong enough to fight it off. That's my understanding of it, at least.
 
1 of my swordtails has a few spots!

But he is acting normally and eating.
The other fish in the tank are also happily swimming.

Should I treat them? or just leave it?

Def. Treat him! Ick will break out worse and affect your whole tank. By now it's probably in the water column
