Cichlid with a wound on his fin

Has anyone seen this issue before, this fish is around 12 years old. He has injured himself somehow.. I have treated with melafix and pimafix to no avail.
if anything the medications have made it worse.

Ive been doing a 50% water change frequent to make sure the water is pristine.

any suggestions on what exactly this is?

Could be bacterial ulcer (or infected now anyway as getting worse). I've not seen much on melafix and pimafix making an infection worse directly. Usually they are first level meds that can't handle serious infections ime.
 
I agree, it does look like a bacterial ulcer. Depending on how badly you wish to save the fish, you could attempt treating with Seachem Kanaplex, a fairly strong antibacterial/fungal medication that WILL demolish your bio-filtration. I have never had to use Kanaplex myself, but I have heard it will handle most bacterial infections. Be sure, if you choose to use it, to do huge water changes daily, because of your bio-filter being destroyed.
