Do you acclimate?
Do you Acclimate your fish?
Make sure that you leave the fish inside the bag and put it in the aquarium water for at least 30 minutes, that way they can get used to the temperature.
Then start adding small amounts of water to the new fish's bag. Half a cup every ten minutes will do it. Do it at least for three times, that way, they will get used to the hardness of your water and the chemicals on it little by little.
After you are done with this, get your fish out of the bag with a net. Don't put all the water from the bag inside the aquarium! This water could contain virus and bacteria that could affect your tank!
Use StressZyme and Stress Coat every time that you add new fish into the tank.
I would also recommend you to use MelaFix, it prevents diseases in newly added fish.
