My betta is very sick!!

I have a 5 gallon tank for him and I came home yesterday from being away for the weekend to him acting weird. He wouldn't eat, he looked thinner, and grey. He didn't swim much only to get some air. Today I bought some ick medication and some brine shrimp (his fav food). I was thinking he maybe had a parasite so I did a 75% water change and added the food and medication. I just came home from work and he looks like this with a white stringy thing coming from his belly. WHAT DO I DO!!!!

He doesn't have ick. He may have internal parasites. ( stringy poo ) ... You may need a medication that treats for internal parasites if it's not too late..
