Possible Fin Rot help/diagnosing

I have a Bala shark with what I'm guessing is fin rot. I've been treating with Primafix and Melafix for a week now and he's only gotten worse. His fins are very jagged now and he does a lot of laying on his side. I moved him to a quarantine tank yesterday but I'm hoping to get other suggestions on what I can do to help him out. (If this sounds familiar, I have a similar thread in the general advice section.)

How big is the shark? How big is the tank? Do you know your ammonia, nitrite, and nitrate levels? The more information you can provide us, the better people can help you here.
