Purple raised spot on a clownfish

The clown has been in a quarantine tank for 4 weeks now. She was treated for itch with copper. The itch has been gone for two weeks. Copper levels are zero. Unfortunately, setting up a QT was an emergency, so it is still cycling. Ammonia 0, nitrite 2, nitrate 10.
I noticed the spot today. It is purple in colour and is raised above the skin level. The tank is bare, except for a submarine toy. The fish is very active, opens its mouth rapidly. There is another little clown, he seems ok.
I have been doing 25% water changes every second day to reduce the stress of bad water. The tank is small, only 40 litters (a QT).
Grateful for your advice.
 
In QT you do water changes to ensure there are no readings of ammonia or nitrite. Ammonia and nitrite are toxic to fish and burn their gills. It is hard to see on that picture, but is likely that this is what you are seeing.
