Added about a 4 inch puffer in my 110g tank on friday. The other 3 fish are small. Never had a ammonia problem. Now my readings for the last two days are. .25 ammonia 0 nitrites 20 nitrates. Should I water change or see if it balances back on its own.

It should balence back out.

It just needs to get use to your new puffer.

I am no expert but it thought puffers were quite aggressive.

Trial And Error?

Hope i helped Aston

If it gets higher than 0.25, then do a water change. Test daily like you're doing now. Feed very little for the first 1-2 weeks. Slowly build up to the regular feeding schedule as your water stabilizes.
