Sick tiger barb

I'm not sure whats wrong with my barb. One of the scales seems to be lifted, the skin under is red, and out of the center is something white. I'm not sure if it's an infection, disease, or injury. It almost resembles a zit. What should I treat her with? Should I leave her be?

Can you post water parameters and a picture? Also how long has the scale been like that?

Everything is at zero except for my nitrates. They are at 30ppm. The scale has been like this for two or three days now.
