Looks like an anubias coffefolia to me. Looks in bad shape indeed. If it is indeed an anubias species, then don't cover the rhizome (it's probably rotting already).
It's salvageable. It looks like there's some squishy that would come off easily with your fingers. If it gives any resistance, leave it. Otherwise, remove it. Take another picture after you do that.
