Sick Pleco!

Pleco is sick, not 100% what is wrong. We love our pleco he's usually very active and now he's not that active and it looks as if he has mucus or some weird growth on his face and mouth! any ideas!

Looks like a water quality issue what are your tank specs... Size and ammonia and nitrate reading?
