Id plant and whats wrong with it?

I've had this plant and it was all right in-till i switched to a fluorescent bulb, I would like to know what type of plant it is.

That looks kind of like the plant that Blitzcraze was asking about.

Where did you get it, how much light do you have, do you fertilize, and what are your nitrate levels?
The white edges look kind of like nitrate deficiency.

We'll i got it at my Lfs which is a really nice one, i believe it is a type of sword with my experience's with plants? I have it in a 10g tank with 2 15w incandescent bulbs and it was doing fine intill i switched it with a new hood with a ( i believe a 8k 18" fluorescent bulb and it started to do that, I do not fertilize, not yet hopefully buying a 20g and putting in some type of substrate, i recentenlly took some testing and my nitrate's were a little high, it was red so about 80, and i know thats not in the safe level so i did a 40% pwc and now it's down to about 20-40ppm
