Mind you I'm not sure which type of ghost shrimp I buy, there are several, they have attacked my fish before. My puffer is plenty healthy too lol. It does happen, it just may not have happened to you yet..

If you actually have ghost shrimp. I'd be far more inclined to believe your puffer attacked the shrimp rather then the other way around.
Ghost shrimp aren't even built in a way to do a healthy fish any serious damage.
