Ancistrus with large stomach?

Any ideas what is going on with this fish? Sick?

Also, is it a male or female?

Looks like a male to me. It appears to be bloat.

Tell your friend to test water parameters first. If the fish has just developed that belly it is most likely bloat and should be treated.
