Great question! I can tell you what I do. I have several large, planted tanks and clean the HOB filters every two weeks when I do my water changes. I like to use HBH pad media. It's inexpensive and works very well. I take the filter apart and clean each piece. Then I take all the media and rinse it in some of the tank water from the water change. If the media is still in reasonably good shape, I just put it back into the filter. If not, I replace the the pads that need it and rinse them in the used tank water.
Everything goes back together and I'm good for two weeks until the next scheduled water change. Pretty easy.
