need advice about my hammer coral

I don't know what's wrong with this coral, I think he might be getting ready to split but idk and need help?

It doesn't look like it's splitting but it doesn't look very happy. Where are all your water parameters at?

My water is excellent and all my other carols are fine including my anemone

Sorry, but could you give some specific numbers? What test kits do you use? We really need more specifics or else there's not a whole lot we can do to help.

What's your salinity? Refractometer or Hydrometer?
What's your water source?
How long have you had the coral?
How was it acclimated?
What lights do you use?
How long has the tank been set up?
What is your water change schedule?
 
I agree it's not looking too good. Not sure what to correct without some specifics mentioned above in regards to perfect water.

I had that hammer for almost 2 months!
it's a 4 gallon pico innovative marine 4 watts led!
tank been set up for 3 months and 15 days
every 3-4 days I change my water
 
Is the circulation pump blasting current on it too much?
