Whatever it is it doesn't look healthy... How long did you acclimate it?

First what are the water parameters that poor fish looks like it seen better days
how long was tank set up and was it cycled what size tank
I can almost guarantee it's ammonia poison
in all reality you shouldn't bring in wild caught fish as they can bring in parasites
the fish looks like a Hardwicke Wrasse and needs a tank of 100g or more

Erm... How'd you catch that fish? Nose looks bloody, probably part of the issue

What are the water parameters?

How long has the tank been set up? What size is it?

It is very difficult to take an injured wild caught fish and keep it in a tank situation.

These fish are used to eating wild fresh live food. Are you able to provide that? Frozen would be the next option, but could take time to get the fish used to that type of food.

Knowing the variety will help decide what kind of diet it should have. It maybe better to return it to where it came from.
