Sick bala

I have a small bala shark that has been in the tank for 2 months. Noticed some missing scales here and there. Thought maybe scraped decoration or something but now had a big ulcer looking mark on the side. Whitish color around a redish centre

I was thinking of adding some salt to the tank, not sure if it will help or if I need meds too. Pic attached. Water is good all zeros with 30 ppm nitrates. Water change just over a week ago.
