Water changes while medicating

Hi, am I supposed to preform water changes while treating my fish? The water test results show ammonia 0.25ppm, nitrite 0ppm and nitrate 20ppm.
 
Most medications will say if water changes should be done or avoided. Which medication are you using?
 
Hi, I'm using interpet anti internal bacteria in my hospital tank. My one platy has blown up like a balloon and my one guppy is real sick, just sitting on the bottom. He has really rapid gill movement. Don't know what to do for the best.
 
I've never heard of that medication but the rapid gill movement could be decreased oxygen. I would do a water change and increase surface agitation and see if that will improve their health
